Covid Pre-screening Questions
I declare that:
- If I’ve travelled outside of Ontario that i have had a negative COVID test upon my return.
- I have not had close contact with anyone in the past 2 weeks with someone who has had a positive COVID test.
Are you experiencing any of the symptoms below?
- Sore throat, hoarse voice, difficulty swallowing
- Headache, runny nose, nasal congestion, sneezing
- Muscle aches, chills, unexplained fatigue/malaise
- Loss of sense of smell, or taste or both
- Conjunctivitis (Pink eye)
Has a medical Provider advised you to self-isolate for COVID-19 risk factors?
I have not knowingly been in in contact with any person with Covid-19.
In-Person COVID Accommodations
Cheat Sheet: What should I bring? A mask (or one will be provided), a travel mug for water during the session (optional).
Please bring your own mask or one will be provided for you. Masks must be worn for energy healing sessions.
The Hamilton Centre for Personal Development (which includes my office) are following all of Ontario’s COVID-19 Health & Safety regulations for cleaning & sanitization.
What If I Don’t Pass the Pre-Screening Questions?
If either you or I do not pass the pre-screening questions, appointments will be online. If I do not pass the pre-screening questions, I will do my best to notify you with as much notice as possible. For those whom an online session is not possible, the appointment will be rescheduled to a later date.
How New COVID Regulations by Public Health affect your Session Confidentiality
For In-person sessions during the COVID pandemic, please note a limit to confidentiality may include the requirement to disclose client names, appointment dates, time of entry and departure to Public Health to aid in contact tracing should a client have a confirmed infection during the course of appointments.
This change in confidentiality limitations is for your safety so that you will become aware of any health & safety issues if they occur at my office. I am happy to answer any questions or concerns you may have.
